Transforming Healthcare at Scale: NHA’s Cloud Migration and Managed Services Journey with Minfy

Project Overview:

The National Health Authority (NHA), responsible for implementing India's flagship public health insurance scheme, Pradhan Mantri Jan Arogya Yojana (PM-JAY), and the Ayushman Bharat Digital Mission (ABDM), engaged MinfyTechnologies to migrate its existing IT infrastructure and application workloads from a private cloud environment to a public cloud environment (AWS). This complex project involved the migration of mission-critical applications supporting PM-JAY, which serves a vast population across various states inIndia. The project encompassed a thorough cloud readiness assessment, application portfolio analysis, migration planning and execution, followed by a transition to a fully managed services model. The migration was executed in waves, starting with a Proof of Concept (POC) before proceeding to full-scalemigration.

 

Challenges:

Scalability Limitations: The existing private cloud infrastructure struggled to efficiently scale to meet the increasing demands of PM-JAY’s applications, impacting the ability to support arapidly growing user base and fluctuating workloads.

High Operational Overhead: Managing their infrastructure on their own, resulted in elevated operational costs and complexities, making it necessary for the NHA to seek a more cost-effective and streamlined solution for resource management.

Limited Monitoring Capabilities: The previous infrastructure was restricted to basic infrastructure and container monitoring, offering insufficient visibility into system performance and potential issues. It lacked advanced features such as distributed tracing, alerting, application performance monitoring (APM), and synthetic monitoring, increasing the risk of service disruptions.

 

Solutions:

Comprehensive Assessment: Minfy conducted an exhaustive assessment to create a tailored migration plan that addressed both the transition from private to public cloud and ongoing public cloud management, considering the scale and complexity of PM-JAY's applications.

Phased Migration: The migration was executed in phases, starting with TMS and HEM, which were successfully migrated across 18 states, followed by the BIS and UMP applications in 22 states. This approach minimized disruption and allowed for iterative improvements.

Proactive Monitoring and Support: A comprehensive monitoring system was implemented using AWS CloudWatch, featuring an executive dashboard to track performance metrics, system utilization, application health, and breakpoints. AWS X-Ray was employed for distributed tracing and application performance monitoring (APM), while CloudWatch application signals ensured effective monitoring of apps and microservices. Additionally, CloudWatch Synthetic Canaries provided proactive synthetic monitoring. This solution enabled continuous oversight of the performance, security, and availability of PM-JAY's cloud infrastructure and applications, ensuring swift resolution of potential issues to maintain seamless service delivery.

Managed Services Model: Minfy's managed services model was crucial to the continuous success of PM-JAY’s cloud operations. This model ensures:

  • Business Hours Availability and Support: A dedicated support team provides dedicated assistance during business hours (percustomer requirement), monitoring systems constantly to pre-emptively resolveissues and ensure high availability.
  • Scalability and Flexibility: Using automated scaling solutions and multi-AZ architecture, Minfy ensures thatPM-JAY applications can adapt to fluctuating demands while maintaining optimal performance. This flexibility caters to both urban and rural beneficiaries, ensuring that services remain accessible regardless of location.
  • Comprehensive Performance Optimization: The managed services framework continuously analyses application performance,identifying and addressing potential bottlenecks, thereby maintaining peak performance across all services.
  • Robust Security Management: The managed services model includes integrated security practices that ensure ongoing compliance with regulatory standards and safeguard sensitive patient health information.

Fraud Analytics Application Development: An infrastructure setup for a fraud analytics application (NAFU) was established, incorporating Redshift, QuickSight, SageMaker, Bedrock, and Lambda functions to enhance data analysis and monitoring capabilities.

Business Outcome:

Successful Migration: The transition of PM-JAY's applications was completed with minimal downtime, ensuring continuity of essential healthcare services.

High Availability Across the Nation: The architecture was designed for robust service availability, ensuring PM-JAY’s applications are accessible to beneficiaries from urban centres to rural areas across India. Redundancy and failover mechanisms were implemented to maintain service continuity.

Enhanced Monitoring and Proactive Support: The establishment of a comprehensive monitoring framework provided real-time visibility into system performance, enabling quick identification and resolution of potential disruptions. This enhanced monitoring capacity ensured critical healthcare services remained consistently available.

Enhanced Security and Compliance: The security posture of PM-JAY was significantly improved, ensuring compliance with stringent regulatory requirements and protecting sensitive patient health information (PHI).

Cost Savings: The transition to a public cloud environment led to substantial cost savings of 25% compared to the previous private cloud, primarily due to reduced operational overhead and more efficient resource management.

Disaster Recovery Metrics: The project established a Recovery Point Objective (RPO) of 45 minutes and a Recovery Time Objective (RTO) of 2 hours, ensuring effective disaster recovery solutions were in place.
